Studienlage für Rivaroxaban (Xarelto®) PIONEER- AF- PCI für Triple- Therapie und Dabigatran (Pradaxa®) Re-Dual- PCI für Dual- Therapie (NOAK+P2Y 12), ISAAR- Triple: 6 Wochen Triple- Therapie
Beachte: Xarelto®15mg+ Clopidogrel 75mg+ ASS 100mg
- bei GFR<50ml/min: Xarelto®10mg, statt Clopidogrel: Prasugrel oder Ticagrelor mgl.
Beachte: Pradaxa® ab GFR <30ml/min. nicht mehr mgl., bei Pradaxa® Dual- Therapie ausreichend!
Studienlage für Apixaban (Eliquis®) AUGUSTUS- Studie für Patienten nach PCI bei ACS und VHF
BEACHTE: normale Dosis Apixaban nach Fachinfo + P2Y 12 (Clopidogrel 92,6%, Ticagrelor) und
nur reduzierte Dosis Apixaban bei VHF, wenn indiziert: 2 von 3 Kriterien:

Alter >=80Jahre, Krea>01,5 mg/dl, Gewicht <= 60kg
2.1. Hohes Blutungsrisiko:
2.1.1. Xarelto® 15mg+DAPT1 Monat-------------------dann Xarelto 15mg+ Clopidogrel 75mg 11 Monate,
im Anschluß NOAK adäquate Dosis entsprechend Indikation, Nierenfunktion, Alter, Gewicht
2.1.2. Pradaxa® 2x110+ Clopidogrel 75mg ---------------------------------------------------------für 12 Monate,
im Anschluß NOAK adäquate Dosis entsprechend Indikation, Nierenfunktion, Alter, Gewicht
- statt Clopidogrel 75mg Ticagrelor 2x90 mgl.
2.1.3.Eliquis 2x5 (2x2,5mg)+ Clopidogrel 75mg------------------------------------------------für 12 Monate,
im Anschluß adäquate NOAK- Dosis
- statt Clopidogrel 75mg Ticagrelor 2x90mg mgl.
2.2. Hohes koronares/ischämisches Risiko, niedriges Blutungsrisiko:
2.2.1.Xarelto®+DAPT 6 Monate------------------------------------dann NOAK+ Clopidogrel 75mg 6 Monate,
im Anschluß NOAK adäquate Dosis entsprechend Indikation, Nierenfunktion, Alter, Gewicht
2.2.2. Pradaxa® 2x150+ Clopidogrel 75mg ---------------------------------------------------------für 12 Monate,
im Anschluß NOAK adäquate Dosis entsprechend Indikation, Nierenfunktion, Alter, Gewicht
- statt Clopidogrel Ticagrelor 2x90mg mgl.
2.2.3. Nach AUGUSTUS offene Dauer + ASS
Eliquis 2x5 (2x2,5mg)+ DAPT (Dauer individuell)----dann NOAK+ Clopidogrel ------------für 12 Monate,
im Anschluß adäquate NOAK- Dosis
